One Chicago hotel was billed by a carrier for calls originating from another company located in the same area. The hotel had no idea it was footing the bill for the other firm’s phone calls, reports Hotel and Motel Management.

An 800-room hotel in a major city, which would normally have qualified for digital service that provides direct answer supervision, was encouraged instead by a vendor to install analog trunks with the vendor’s own answer-detection equipment. The digital trunks were less expensive and would have provided better service with more advanced technology.

For smaller hotels this may not result in a loss of a lot of money, but at business properties with a good activity level, telecommunication problems can add up to significant costs. A hospitality call accounting system can help hotels and motels with telephone call billing and profit reports accurately generated on a real-time basis. Reconciliation of call accounting records with the property management system saves hundreds of hours. A quality hotel call accounting system should offer the storage of call records beyond the normal 24-hour audit period, typically 60 days of calls. It should also be able to generate a guest bill if the property management system is down for an extended period of time and the night audit report should match calls and charges that have been sent to the PMS. Also, look for telephone call profit reports that are accurately generated on a real-time basis. Reconciliation of call accounting records with the property management system saves hundreds of hours.

Commonly available is a special rating table operation that enables calls that have been placed to newly created area and office codes to be billed, even though they have not yet been programmed into the rate tables. This insures call records are priced and stored for guest checkout or sent to the property management system (PMS) and billed to a guest folio. Capturing and storing all call detail records allows management to oversee both guest and employee phone usage and full trunk usage, thus allowing the best management of long distance carriers, the number of trunks needed and reconciliation of phone bills.

Prostate cancer as one of the most common types of cancer in men according to the American Cancer Society. Fortunately if caught early, the prognosis is good. Even better than treating it, is of course, avoiding it altogether. The search for the cause of prostate cancer is ongoing and a recent study was recently completed.

An eight year study of 29,361 men has just been completed. The study examined the impact of antioxidant supplements on the rate of prostate cancer. “There has been definite interest in their use, based on a few earlier studies that have been done” said lead researcher Richard B. Hayes, senior investigator in the division of cancer epidemiology and genetics at the U.S. National Cancer Institute.

Reporting in the Feb. 15 issue of the Journal of the National Cancer Institute, Hayes’ team calculated the risk of prostate cancer for 29,361 men aged 55 to 74, all of whom were participants in the Prostate, Lung, Colorectal and Ovarian (PLCO) Screening Trial.

Some of the data that was analyzed included the amount of common antioxidant supplements including vitamin C, beta carotene, and vitamin E. Some of the men had been taking these supplements for many years prior to the study.

Over the total eight year study period, 1,338 men developed prostate cancer. The results indicate that the use of antioxidant supplements had no positive impact on the risk for prostate cancer for most nonsmokers. Beta carotene supplements did have a positive impact among men that had low dietary intake of beta carotene.

Vitamin E did have a positive impact for smokers according to the research. This was consistent with previous trials.
Although the three supplements studied in this trial showed little help in preventing prostate cancer, previous trials that examined other supplements did show very encouraging results.

Fish oils, which contain EPA and DHA, were found to reduce prostate cancer by 11% in men that consumed about 470 mg./day. Selenium has been found to cut prostate cancer in half. Over five clinical trials have supported this. It is recommended that men take a 200 micrograms of selenium daily.

“low fee” I could get a M.A. in only five days - no studying, classes, or even a highschool diploma is

required. Another email “invited” me to a birthday party for the CEO’s son, with party favors including a

free 30-day trial for their web hosting service.

If you use either AOL or AT&T for your Internet service, you have free access to high-quality, ad-free web

space. You don’t have to pay an extra fee or go through the hassle of registering for free web space

because it’s already included in your service plan.

For AOL users, enter AOL keyword: FTP. AOL provides five megabytes per username, totaling 25 megabytes.

Your web address will be http://members.aol.com/AOLusername.

For AT&T users, visit http://home.att.net AT&T gives you two addresses

I ranked 20+ free web hosting companies from tremendous to terrible. Using a point-based system, I added a

point to their score for every positive service they provided, and each negative service they provided

(such as displaying ads on your site) deducted a point from their score. (Maximum possible points a

company could earn was 20 points) After all this was done, two companies stood at the top of this list -

whose peerless scores far surpassed the scores of any other company! Earning 18 and 14 points

respectively, these two companies far surpassed even major companies like YAHOO! (earning 4 pts), Lycos (6

pts), and even Microsoft (5 pts)!

If you desire an easy to use hosting service, Portland Communications is the best provider for you. While

FreeWebs provides many attractive services, it seems to forget the “basic necessities.” (like being able

to upload more that one file at a time)

Placing second with a hit counter and 15 megabytes of ad-free space, Portland Communications has a few

distinct advantages over FreeWebs.

First, Portland provides a much more favorable URL: http://yoursite.port5.com. It is much easier to

remember, and it even appears that you have your own domain name!

Second, you can upload multiple files at once. FreeWebs only allows you to upload one file at a time,

which I found to be a very tedious process. Portland allows FTP access, which means that you can simply

select the files you wish to upload and return when the process is completed.

Note: FTP may be somewhat tricky for the beginner, so you may want to use FreeWebs or Portland, depending

on if you are comfortable with using FTP. I’d suggest trying both of them, and see which on you like

better. If you decide to use Portland, you can purchase a great FTP client (or download a free trial) at

http://xenolth.biz/ftpclient. If you’d like to use a free FTP

client, you will find one at http://xenolth.biz/freeftp.

If you want to be able to upload your website without a hassle, Portland is the hosting provider for you.

service. Note: although the server is in the UK, your site will have a “.com” address.

Fast Url

Some of the hosting services mentioned above provide URLs that are hard to remember. Instead of trying to

make your visitors remember such a cumbersome URL, you can get a Fast URL like “clix.to/yoursite.” You can

even chose from multiple “.to” addresses. To sign up for this free service, visit http://clix.to/fasturl,

then click “Web Tools and Services,” then click “Fast URL Redirect.” You must have a website before you

can sign up for Fast URL.

General Note: When uploading your website, make sure to name your homepage “index.htm” or

“index.html.” This is because the only way the server knows which page is the homepage is by looking for a

file named “index.”

Doctor job - Hospitalists: Can You “Catch” One In The Hospital?

Recent research by a New Zealand University team into colon cancer has uncovered a very interesting phenomenon that sheds light on why colon cancer is almost non-existent in the Maori race. Naturally this fact created a spin-off from the original study because if the reason why the indigenous Maori people avoided this disease could be found they would gain precious information to help the constant fight to prevent colon cancer.

Initially they looked at diet and discovered that both red and purple berries and fruits formed a higher proportion of their diet than it did with non-indigenous New Zealanders. Based upon common practice they were aware of the anti-oxidant value of fruit and assumed that non-Maori people gained the same amount of anti-oxidants from other fruit and vegetables. This appeared to cancel out any benefit the Maoris gained by having a diet high in red and purple fruits and vegetables. That was until they decided to check anti-oxidant levels individually.

The results of the independent checks of anti-oxidant levels across a wide range of fruits and vegetables discovered that rather than these levels being the same, as commonly thought at the start of the experiments, they vary widely. For instance fruits with red or purple skins like berries, plums, red apples and even red skinned sweet potato have around four times the anti-oxidant levels of other fruits and vegetables. At this point the entire project started to make sense and the higher levels of ant-oxidants in a traditional Maori diet pointed to the reason for such low levels of colon cancer within the race.

The research continues today but based upon these findings there is strong reason to eat more strawberries, raspberries, redcurrants, cherries, plums, red apples and sweet potato in our daily diets. In fact any fruit or vegetable with a red or purple skin contains around four times the anti-oxidant level of other fruits and vegetables and therefore should be utilized in our diets for health reasons.

Somebody is bound to raise the question of whether frozen is as a good as fresh fruit. Here I can only give a personal opinion and state that whenever we cook, or freeze, foodstuffs we change the chemical make-up of the food and therefore it is unlikely to be the same as eating it fresh and uncooked. But, if frozen is all that is available then I guess its better than nothing.

It’s an emerging trend, and one that many patients find confusing and uncomfortable: their primary doctor doesn’t visit them in the hospital any more and doesn’t manage their hospital care.

Primary care doctors are increasingly turning the care of their hospitalized patients over to specialists called “hospitalists.”

The hospitalist is a hospital-based doctor who does not see patients in an office-based practice. He or she manages the care of patients only while they are in the hospital, turning them back over to their regular physicians when they are discharged. During the time a patient is in the hospital the hospitalist is responsible for all decisions about a patient’s care.

Advantages of Hospitalists

The hospitalist usually knows the hospital, and hospital politics, very well. This often enables the hospitalist to cut through red tape and “make things happen” more efficiently than office-based physicians.

Hospitalists are more readily available to respond to emergencies in the hospital. Nurses and other care staff can usually reach a hospitalist more rapidly than an office-based physician, especially on evenings and weekends.

Continuity of care within the hospital is often better. When primary care physicians manage inpatient hospital care, the patient is often actually seen by more doctors, as doctors in larger practices often take turns seeing all of the practice’s hospitalized patients.

Hospitalists are usually more accessible to family members. Families don’t have to try to “catch” the doctor in the wee hours of the morning or late in the evening when he or she is making hospital rounds outside of office hours.

Disadvantages of Using a Hospitalist

The biggest disadvantage to the movement toward hospitalists is the loss of continuity of care between the primary physician and the hospital. The hospitalist has no previous knowledge of his new patient. If communication between the primary care physician and the hospitalist is poor, it falls to the patient and the family to fill in the gaps.

When a patient is discharged from the hospital the hospitalist relinquishes care back to the PCP. If communication has not been good, the primary care physician often has little knowledge of what the patient experienced in the hospital. Records are frequently slow to follow the patient, so on the first follow-up visit the office-based doctor may have scant information.

Without adequate information the PCP frequently makes adjustments to treatment plans and medications that are counter to the treatment plans initiated in the hospital.

How To Work With A Hospitalist

Be prepared. If your trip to the hospital is pre-planned, talk to your primary doctor about the hospitalists in your chosen hospital. Find out which hospitalist communicates best with your doctor, and who your doctor prefers to work with. If possible, ask your doctor to pre-arrange that this hospitalist will be in charge of your care while you’re in the hospital.

Arrive Armed: Never assume that things will go as planned. You may not feel well, and you will be under stress. The chosen hospitalist may be on vacation, out ill, or unavailable. Take a synopsis of your medical history, including the results of all recent tests, with you to the hospital. Whenever possible have someone stay with you in the hospital until you have met your doctor and given him or her all the information you have. Have that person take notes, including the names of all your hospital caregivers, their contact information (phone and pager), and the location of their offices in the hospital.

Sign a Release: If you can, make sure to sign a release of information form when you are admitted. This will give the hospitalist and everyone else on your medical team permission to discuss your care with the person you appoint. Even if you have already given that person your Power of Attorney for Health Care, signing a permission to release information is a good idea. You might not be so ill that your Power of Attorney for Health Care goes into effect. In that case, without authority to release information, you might find the staff is less than willing to share information with the person staying with you.

space. You don’t have to pay an extra fee or go through the hassle of registering for free web space

because it’s already included in your service plan.

For AOL users, enter AOL keyword: FTP. AOL provides five megabytes per username, totaling 25 megabytes.

I ranked 20+ free web hosting companies from tremendous to terrible. Using a point-based system, I added a

point to their score for every positive service they provided, and each negative service they provided

(such as displaying ads on your site) deducted a point from their score. (Maximum possible points a

whose peerless scores far surpassed the scores of any other company! Earning 18 and 14 points

If you desire an easy to use hosting service, Portland Communications is the best provider for you. While

FreeWebs provides many attractive services, it seems to forget the “basic necessities.” (like being able

to upload more that one file at a time)

Placing second with a hit counter and 15 megabytes of ad-free space, Portland Communications has a few

distinct advantages over FreeWebs.

First, Portland provides a much more favorable URL: http://yoursite.port5.com. It is much easier to

remember, and it even appears that you have your own domain name!

Second, you can upload multiple files at once. FreeWebs only allows you to upload one file at a time,

which I found to be a very tedious process. Portland allows FTP access, which means that you can simply

select the files you wish to upload and return when the process is completed.

Note: FTP may be somewhat tricky for the beginner, so you may want to use FreeWebs or Portland, depending

on if you are comfortable with using FTP. I’d suggest trying both of them, and see which on you like

better. If you decide to use Portland, you can purchase a great FTP client (or download a free trial) at

http://xenolth.biz/ftpclient. If you’d like to use a free FTP

client, you will find one at http://xenolth.biz/freeftp.

If you want to be able to upload your website without a hassle, Portland is the hosting provider for you.

Some of the hosting services mentioned above provide URLs that are hard to remember. Instead of trying to

make your visitors remember such a cumbersome URL, you can get a Fast URL like “clix.to/yoursite.” You can

even chose from multiple “.to” addresses. To sign up for this free service, visit http://clix.to/fasturl,

then click “Web Tools and Services,” then click “Fast URL Redirect.” You must have a website before you

can sign up for Fast URL.

General Note: When uploading your website, make sure to name your homepage “index.htm” or

“index.html.” This is because the only way the server knows which page is the homepage is by looking for a

file named “index.”

Because of the advanced stages of some lung cancer, surgery is not an option. This depends on the location of the tumor/tumors or the overall health of the person. In these types of cases, an oncologist will most likely advocate radiotherapy for the treatment. Even though surgery may be an option, you may find the doctor suggesting this type of treatment either before surgery to help shrink the tumor or after the surgery to help eradicate any of the remaining cancerous cells.

Radiation therapy - what is it? Radiotherapy - is high powered x-rays that are ‘beamed’ at the location of the cancer cells which in turn destroys the rapidly dividing cells. Unfortunately, during this process the radiotherapy kills off both the healthy cells as well as the cancerous cells in that location. However, there are methods of delivering the radiotherapy that will minimize the side effects on the healthy cells.

EBT - external beam therapy or radiotherapy, is a highly focused beam of radiation that is aimed directly at the edges of the tumor site. Due to the concentration, EBT minimizes the chance of damaging the healthy cells. EBT is delivered in ‘fractions’, i.e. short durations of radiation that are spread out over several weeks time. According to current studies, the best effects are achieved when there are more frequent, shorter fractions than in larger fractions spread out over a longer period of time. Talk with your doctor about which will be the best timing for you.

Often times this therapy will be used to alleviate symptoms even though it won’t actually cure by itself. Doctors may prescribe radiotherapy before or after a surgery and is designed to shrink the tumor or make it less active. Doctors may suggest brachytherapy instead of using an external beam. Brachytherapy is a procedure which implants small radioactive seeds directly at the site of the cancer. Brachytherapy can open up blocked airways by reducing the size of the tumors blocking the bronchi or windpipe. Brachytherapy can also help stop bleeding in tumors.

Conformal radiation therapy is another type of therapy. With this therapy a CAT scan helps create a 3-D image of the tumor and shape the x-ray beams precisely to the size and shape of the tumor. When using conformal radiation therapy, the damage to the surrounding tissues is nearly eliminated completely. Intensity modulated radiation therapy can be fine tuned so that it only affects certain areas of the tumor. This form of precision therapy works with a CAT scan to target only the cancerous cells.

Any suspicion that the tumor may have spread, as is often the case with small cell, your oncologist will very likely suggest a course of chemotherapy either before, after or concurrently with radiotherapy. Radiotherapy is localized therefore the beam is focused directly at the cancerous cells. Using radiotherapy in conjunction with chemotherapy, will normally offer a better chance of clearing up the tumor completely and preventing a recurrence. Radiotherapy is rarely recommended as the sole treatment.

If someone you know and love is in the hospital there are a few things you need to know especially if you are the Heath Care Agent or the surrogate that will be acting and making decisions for the patient.

Keep a notebook and a pen available with you at all times. You will have many things going through your mind that you may need to keep track of and the notebook will be helpful. Much of this information will be exchanged in the days to come with physicians, other family members and friends, and the discharge planner. Family or friends may be asking you questions and offering to do things for you. You do not have to do all of the writing, assign whoever is with you to do some of the writing for you.

Be sure to use the opportunities that family and friends will be offering you now. Select someone you trust who is supportive to be your helper. Let that individual know what it is that you need for them to do. Several others will be offering to help as well, but for the next few days you need a support person. There will be those that offer to do specific things for you, allow them to do those things as long as you trust them. Then, there will be other individuals that will ask if there is anything that they can do for you that is non-specific, if know what these individuals can do for you, go ahead and ask them to do it. Make the most out of this opportunity to allow others to give to you when they offer. (They may not later).

Find out who the discharge planner is and schedule a meeting as soon as possible. Discharge planning is a process and it begins at admission. The discharge planner will help you arrange the care needed for your loved one or friend when it is time for them to go home. They must notify you of their plans before the discharge occurs. The discharge planner can also be your liaison to get needed information from physician and the different specialists who are following your loved one or friend.

Ask the nurse when the physician will be doing his rounds. Most physicians do daily rounds or have rounds assigned. Be there during rounds. Make the most of these daily opportunities. This will be the time that you can ask the physician questions and set up a meeting to talk about your loved ones condition, progress, diagnostics, treatments, medication and what may happen in the future.

Contact your insurance company to find out what is covered or not covered in your policy. Get the name of a contact person and their assistants name for future questions that you will have for them about rehabilitation, home care, nursing home care, etc. If, you have Medicare, the discharge planner or admissions office will be able to help with Medicare coverage while your patient is in the hospital.

You are the advocate now. Do not be afraid to ask questions. Every question you ask and receive answers to will help you make better decisions about the care your loved one will need. No matter what it is that you do not know or understand about the care that your loved one is receiving, ask. Some professionals forget to break down their knowledge in to terms that are easy to understand. Ask them to simplify or use laymans terms.

Take care of yourself. Make sure that you are drinking adequate fluids including water. Be sure to have your regular meals and dont rely on snacks to keep you going. You need rest. Try to take small naps when your loved one is napping even if it is at bedside. Use a Do Not Disturb sign if you need to. Remind others when you need privacy. You may require emotional support. The hospital has a Chaplin and social workers available that can offer you support and counseling.

Take some time to think about how you may or may not be able to care for your loved one. Think about the treatments your loved one is receiving and if the treatment is one that you can learn to do. How will you help your loved one with tasks of activities of daily living? Will you be able to help them walk, transfer, toilet, dress, bath, and eat if that is the help they need? You may not be able to some of these tasks, you may require help, training or maybe a caregiver. You may have to work. Your loved one may require more treatment or rehabilitation to be able to come home. In any case, you should carefully evaluate your circumstances and meet with the discharge planner and let him/her know what help you may need and find out what alternatives are available.

It’s an emerging trend, and one that many patients find confusing and uncomfortable: their primary doctor doesn’t visit them in the hospital any more and doesn’t manage their hospital care.

Primary care doctors are increasingly turning the care of their hospitalized patients over to specialists called “hospitalists.”

The hospitalist is a hospital-based doctor who does not see patients in an office-based practice. He or she manages the care of patients only while they are in the hospital, turning them back over to their regular physicians when they are discharged. During the time a patient is in the hospital the hospitalist is responsible for all decisions about a patient’s care.

Advantages of Hospitalists

The hospitalist usually knows the hospital, and hospital politics, very well. This often enables the hospitalist to cut through red tape and “make things happen” more efficiently than office-based physicians.

Hospitalists are more readily available to respond to emergencies in the hospital. Nurses and other care staff can usually reach a hospitalist more rapidly than an office-based physician, especially on evenings and weekends.

Continuity of care within the hospital is often better. When primary care physicians manage inpatient hospital care, the patient is often actually seen by more doctors, as doctors in larger practices often take turns seeing all of the practice’s hospitalized patients.

Hospitalists are usually more accessible to family members. Families don’t have to try to “catch” the doctor in the wee hours of the morning or late in the evening when he or she is making hospital rounds outside of office hours.

Disadvantages of Using a Hospitalist

The biggest disadvantage to the movement toward hospitalists is the loss of continuity of care between the primary physician and the hospital. The hospitalist has no previous knowledge of his new patient. If communication between the primary care physician and the hospitalist is poor, it falls to the patient and the family to fill in the gaps.

When a patient is discharged from the hospital the hospitalist relinquishes care back to the PCP. If communication has not been good, the primary care physician often has little knowledge of what the patient experienced in the hospital. Records are frequently slow to follow the patient, so on the first follow-up visit the office-based doctor may have scant information.

Without adequate information the PCP frequently makes adjustments to treatment plans and medications that are counter to the treatment plans initiated in the hospital.

How To Work With A Hospitalist

Be prepared. If your trip to the hospital is pre-planned, talk to your primary doctor about the hospitalists in your chosen hospital. Find out which hospitalist communicates best with your doctor, and who your doctor prefers to work with. If possible, ask your doctor to pre-arrange that this hospitalist will be in charge of your care while you’re in the hospital.

Arrive Armed: Never assume that things will go as planned. You may not feel well, and you will be under stress. The chosen hospitalist may be on vacation, out ill, or unavailable. Take a synopsis of your medical history, including the results of all recent tests, with you to the hospital. Whenever possible have someone stay with you in the hospital until you have met your doctor and given him or her all the information you have. Have that person take notes, including the names of all your hospital caregivers, their contact information (phone and pager), and the location of their offices in the hospital.

Sign a Release: If you can, make sure to sign a release of information form when you are admitted. This will give the hospitalist and everyone else on your medical team permission to discuss your care with the person you appoint. Even if you have already given that person your Power of Attorney for Health Care, signing a permission to release information is a good idea. You might not be so ill that your Power of Attorney for Health Care goes into effect. In that case, without authority to release information, you might find the staff is less than willing to share information with the person staying with you.

What’s the monthly premium?
What are the choices for deductibles?
What percentage does the insurer pay after the deductible is met?
What coverages are included in the policy?
Does the policy cover prescription drugs?
Does the policy cover preventative care?
Are your doctors and health care facilities inside of the policy’s network?
What is the cost for seeing a physician outside of the policy’s network?

While you will certainly think of more questions to ask potential insurers, this list should get you started. Remember, the more you know, the better decisions you’ll make!

Protecting Yourself

As with most areas of business, there are a few untrustworthy insurers out there who ruin things for the good guys. That’s why it’s a good idea to investigate your insurer and his or her credentials before signing a health insurance policy.

You can check your agent’s credentials, as well as the company’s customer satisfaction rating and financial standing through consumer-serving sites like AM Best or the Better Business Bureau. Doing a little behind the scenes work on your insurer beforehand will save you the stress and financial loss of doing business with a dodgy insurer.

You can also protect yourself by knowing your rights and privileges before signing on the dotted line. Most states now require insurers to cover certain benefits, such as mammograms and prostate exams; your local DOI will have more information on mandated benefits and other rights and privileges.

Most insurers will also offer a free-look period of 10 or so days to review your policy and make any final adjustments or decisions. If you decide you don’t want the policy during that free-look period, you are entitled to a full refund and your policy will be cancelled without penalty. As a general rule, if your health policy doesn’t contain a free-look period, you’re probably better off purchasing health insurance from someone else.
